What is the difference between case management and service facilitation?

Case management services are provided to all DD Waiver clients. It involves developing, coordinating, implementing, monitoring and modifying the Plan of Care with the individual and his/her family. Linking individuals and their families to community resources, and other supports. Coordinating DD Waiver service providers and monitoring the quality of care the individual receives.

Service facilitation is provided only to those DD Waiver clients who choose consumer-directed services (i.e. attendant care, companion care, and/or respite care) through the DD Waiver. In the Service facilitation role, the Arc staff is responsible for:

  • providing training and skill development opportunities as requested and/or needed that help you be successful as an employer;
  • securing authorization for your consumer-directed services and notifying you when they are approved and you can officially move ahead; and,
  • The individual has behaviors which present risk to personal or public safety.
  • checking in with you at a minimum every three months to see how things are going with your consumer-directed services, ensure that the services provided are authorized and appropriate, and provide you with information as needed for your continued participation in the DD Waiver Consumer-Directed Services Program.
