What is the difference between case management and service facilitation?

Agency-directed care is care that is rendered by an agency approved to be a Medicaid provider. The agency takes care of the hiring and management of the attendant care, companion care, and/or respite care workers. This includes submitting the paperwork necessary for payment to DMAS.

Consumer-directed care involves attendant, and/or respite care workers that are directly hired and managed by the individual receiving the DD Waiver support services. Under this arrangement, the individual, or their family if the client is a child or is unable to manage the task, is responsible for managing all aspects of the employment relationship, including recruiting, hiring, training, supervising, ensuring payment through DMAS, and firing if needed.
